First Experience with Canaloplastyin Lithuania

Purpose: To study first clinical results of viscodilation of Schlemm’s canal with subsequent permanent streching of trabecula with suture (canaloplasty).
Methods: This study included 5 eyes with OAG; one eye underwent trabeculectomy one year ago. Preoperative intraocular preasure (IOP) was 17,3 mmHg or higher. After nonpenetrating external opening of Schlemm’s canal, a flexible microcatheter (iTrack 250A, iScience Interventional) was introduced with injection of Healon GV circumferentially and a suture (double 10 –0 prolene) loop was placed into the canal to stretch the trabecular meshwork permanently. In two eyes cataract surgery was performed simultaneously. In one eye circumferentional probing was impossible because of canal obstruction, therefore canal was dilated 360° and filled with Healon GV. In one eye after trabeculectomy a viscocanalostomy was performed.
Results: No surgicaly induced complications occured. IOP droped down in all eyes (from 17,3 to 15,9 mm Hg) one week after surgery (without medications). Two weeks later we observed elevation of IOP (29 mm Hg) in one eye (combined procedure). This may be due to a steroid reaction.
Conclusions: Canaloplasty seems to be an effective IOP lowering nonpenetrating glaucoma procedure. For more detailed conclusionsa bigger number of cases and longer follow-up is necessary.
